Post 3 of 5. Debugging time. After some inspection and probing it looked like the bottom switch was not reliably being pressed when the shifter was in park. I could have bent the arm of the switch up but I opted to just replace the switch while I had the opportunity.

Post 4 of 5. The new switch fixed the issue. The part number is OMRON SS-01GL13-AQ6 and it’s less than $2.
